Submit your enrollment interest

To begin, complete our contact form below and upload the necessary information—or reach out directly to Christian Maronian, Director of Operations, at (909) 554-4575. We’ll guide you through the next steps whether you’re an individual clinician, part of a group, or a facility.

Gather required information

To move forward, please have the following ready:

  • Corporation W9 

  • Provider Roster

    • Updated & Attested CAQH# for each Provider containing:

      • CA License In Good Standing​

      • DEA Certificate

      • Malpractice Insurance Certificate

      • CV

      • Board Certifications (if applicable)

In lieu of CAQH, our Paper Credentialing Application is acceptable.
Please complete for each provider on your roster and email to
Physicians@EmpireCare.com.

Sign your provider contract

You'll meet with our business team and receive a contract for review and signature. We'll support you throughout to answer any questions and ensure clarity.

 

Complete credentialing process

Once your contract is signed, our credentialing team will verify your credentials, often leveraging CAQH where available, to minimize redundancy and speed approval.

Welcome & onboarding

Upon execution of your contract, you’ll meet with your Account Manager & receive a welcome packet—with instruction to access our provider portal containing resources and essential tools to get you started.
