Become a patient 

At Empire Healthcare, care starts with your doctor—and stays with you. Joining our network means gaining a trusted primary care physician (PCP) supported by a community of specialists and health plans, all working together to keep your care personal, local, and connected.
 

  1. Confirm your health plan. 
    Empire Healthcare partners with leading health plans—including commercial, Medicare Advantage, and Medi-Cal options. First, check that your health plan includes Empire Healthcare in its network.
     

  2. Choose your primary care physician (PCP). 
    Your health plan will ask you to select a PCP. This doctor will be your main point of care—someone who knows you, coordinates your health needs, and connects you to trusted specialists. Find an Empire-affiliated PCP who fits your needs and location below.

     

  3. Review your welcome materials
    Once enrolled, you’ll receive a new member packet with important information about your benefits, resources, and how to access care. Keep your health plan ID card handy—it will list your doctor and medical group.
     

  4. Connect with our team
